The nose is a pyramidal structure found on the middle third of the face. The nose comprises skin, cartilage, bone and mucous membranes.
The upper third of the nose is osseous. The middle and lower thirds are cartilaginous structures.
These structures are covered by skin.
The internal part of the nose is divided into two by the nasal septum, which is part bone and part cartilage.
There are three osseous structures on both

sides of the nose called conchae.
The facial bones form four air-filled spaces known as the paranasal sinuses, which open toward the nasal cavity.
All internal nose structures, the nasal septum, the conchae, the paranasal sinuses and all other upper airway organs are covered by a soft tissue known as mucous membrane.
This mucous membrane secretes mucus that maintains the moisture of the air passing through the airways.
The mucous membrane is covered in delicate, hair-like structures known as cilia.
These structures are crucial to the nose’s function as they provide for optimal discharge of the mucus.
The mucous membrane can heat up or cool down the air entering the nose to body temperature within a fraction of a second, and to bring its
relative humidity to around 70 – 80% thus regulating both temperature and moisture to the body’s internal and natural environment.
Any disruption to one of the nose’s internal structures, the septum, the conchae, the sinuses or the mucous membrane will result in impaired function and improper breathing, and sometimes the problem is so severe that surgery is the only way to correct it.

Rhinoplasty requires proper, meticulous preparation in order to achieve the best results, both aesthetically and in terms of function.

Good communication between the surgeon and the patient wishing to undergo rhinoplasty, working together to define the issues at hand, and a clear and accurate explanation about possible results and undesired side effects – will all help the patient better understand the nature of the procedure, its benefits and limitations, and to adjust expectations accordingly.
Setting expectations is a critical preoperative step.
This will help during emotional recovery and when gauging satisfaction. Address the following before the surgery:
1. Patient’s age, medical history, previous illnesses or operations, allergies to medication, smoking, regular medications, sensitivities to medication and anesthetics.
2. Appearance of the nose before surgery, type of skin, injuries, scars and other external or internal deformities.

Simulation

Cheap and aggressive sales tactics, fraudulent marketing, deception, charlatanism, fake – these are some of the ways that renowned and reputable surgeons describe those providing simulations in an attempt to influence and convince patients to undergo rhinoplasty or any other plastic surgery.
Simulations have no real value, they fail to provide honest and truthful information about the results, and they do not show, nor do they even mention, any possible issues with appearance or function that may occur after the surgery.
In the end, simulations are performed by simple computer software, while surgery is performed by a surgeon whose experience and skill level determine the outcome almost absolutely.
Aside from fulfilling our natural inclination to “see” the results and the false peace of mind that goes along with it, simulations do not provide truthful information, and they certainly fail to show any failures that may occur during surgery, and no surgeon, especially those who provide patients with simulations, will ever sign off on the simulation being the final result.

Most rhinoplasty procedures are performed under local anesthesia of the nose only.
However, since sedatives and analgesics are administered preoperatively and during the operation, in most cases the patient will not feel any pain and will often have no recollection of the procedure.

Rhinoplasty is one of the most demanding and complex plastic surgeries.
It requires the surgeon to be very experienced and skilled.
It is the art of sculpting raw tissues, each with its own unique properties and reactions that cannot be defined or foreseen, but which all greatly impact the final
result.
Rhinoplasty is performed through the nostrils (closed rhinoplasty) with no external incisions on the skin, making it quite difficult to see what is happening during surgery.
The procedure takes between 30 – 90 minutes to complete, during which the bones, cartilage, mucous membranes and skin are all treated. Upon completion, the internal incisions used to perform the surgery are sutured and the nose is immobilized using a plaster or plastic splint for several days.
An otolaryngologist (ENT) is sometimes present during surgery to correct any breathing disorders.

Recovery starts as soon as the surgery has been completed, and is often characterized by the following:
1. Swelling and bruising – Rhinoplasty is accompanied by swelling and bruising (blue marks) around the eyelids. Since every person reacts differently, the degree and severity vary from one patient to the next. Swelling may get worse over the first one-two days after surgery, but it will then gradually subside. You can reduce swelling by cooling the eyelids and lying down with your head and upper body propped up using pillows.
2. Pain – Slight pain during the first day after surgery is normal.
This pain can easily be alleviated by taking regular pain relievers, such as Advil.
3. Bleeding – Slight bleeding from the nose usually lasts for about 24 hours after surgery.
If bleeding does not stop or becomes excessive, contact your surgeon immediately.
4. Stuffy nose – This is usually the result of scabbing and blood clots accumulating in the nostrils.

In any case, avoid picking your nose after surgery despite the nagging sensation as it may cause bleeding and even impact the outcome.
5. Postoperative healing – The healing process is important for the success of any surgery and is a critical factor in determining the outcome.
Thousands of rhinoplasty procedures are performed each year with good results, though occasionally improper healing causes undesired outcomes that must then be corrected.
The healing process is regulated by internal body mechanisms that cannot be adequately defined and anticipated.
The nature and quality of these processes vary from one person to another and that is why different people heal differently.

The results of your surgery are determined by three main factors:

1. The shape of the nose, the type and quality of tissues prior to the surgery.
2. Postoperative healing processes.
3. The surgeon’s experience and skill level.
First impressions of the outcome can be seen when taking off the splint or around two-three weeks later, but the final result can only be gauged once the healing process is complete.
This is a natural process that occurs after surgery and may last several months.
In the overwhelming majority of cases, rhinoplasty significantly improves the shape of the nose and facial beauty, and contributes greatly to the patient’s sense of self and quality of life.

There are two types of postoperative side effects:
Aesthetic
1. Irregular bone and cartilage structure visible through the skin.
2. Swelling in the area above the tip of the nose (due to excessive connective tissue / scarring of the area).
3. Deviation.
4. Asymmetrical nostrils.
5. Scarring.
6. Pigmentation of the skin around the nose or lower eyelids.
Functional
1. Difficulty or irregular breathing through the nose. This is rare and can usually be corrected
through surgery. It may present in patients who suffered from slight breathing issues before surgery which were then exacerbated after the procedure.
2. Changes to sense of smell (very rare).

Side effects following rhinoplasty are very rare and most can be treated.
