The breast is one of the defining characteristics of a woman’s femininity and sexuality. Fuller, firmer breasts are adored by both men and women alike, though desired breast size and correct body proportions greatly divide opinion.

Small breasts are a source of disappointment, frustration and discontent for many women. Women with small breasts often feel that there is something wrong with their sexuality and femininity.

The first breast augmentation surgeries were performed in the 1960s. Over the years, breast augmentation procedures have become increasingly popular in Israel and around the world.

Breast augmentation is currently the most sought-after plastic surgery procedure in many countries and in Israel.

The Breast (Anatomy)

The female breast is made of glandular tissue, fatty tissue and connective tissue.
The breasts start developing when a woman reaches adolescence and continue their development until she is around 17 – 18 years of age, though changes in size and shape often occur later on as a result of biological and hormonal changes in the woman’s body.
The breasts usually swell and become fuller and bigger a short while before each menstruation period, due to the effects of estrogen and progesterone.
During pregnancy and lactation, the breast gains even more volume, becoming firmer and perkier. When lactating, the breast swells, becomes dense and secretes milk, but as soon as lactation ends the breast decreases in size and its appearance changes altogether.

When a woman enters menopause, the breast loses volume, begins to sag and loses its vitality. Breast size is also heavily affected by weight gain and weight loss.

Shapes

Breasts can be either big or small, saggy or loose. The size and shape of the breasts and areolas may also be asymmetrical.
Undesired phenomena can also be observed in men, and sometimes surgical intervention is also required.
Men may also suffer from large, swollen breasts, a phenomenon called gynecomastia, and it presents in either one side or both.
Breast surgeries give the breast a much nicer, more adequate shape and size, but they do not support the breast, nor do they prevent sagging over time.

Implants

The technology used to manufacture implants has developed and improved over the years.
When performing breast augmentation procedures, the surgeon uses a special implant designed to increase the size of the breast and give it more volume and shape. The vast majority of implants used for breast augmentation are made of a silicone shell, containing either saline or silicone gel. Saline implants have not really proven themselves over time and are almost entirely out of circulation in Israel. Silicone gel implants have become the most popular implants and are

currently widely used. Silicone gel implants have various degrees of viscosity. The implants used today are called cohesive gel implants, and they come with a relatively high viscosity.
An important stage in the development of breast implants was the introduction of the anatomical implants, which uses a strong cohesive gel to form a tear-drop appearance.
The effectiveness of these implants is highly disputed among surgeons, especially for breast augmentation procedures.

Size, Shape and Weight

Choosing the right size, shape and weight of the implant is a difficult and complex process that requires great skill and experience.

The size, weight and shape of the implant is crucial to the final result, the shape of the breast, its size and its proportions relative to the rest of the body. When choosing which implant to use for breast augmentation, the surgeon must fully understand the woman’s desires and aspirations, correctly identify the potential her body and breasts have and accurately recommend the most suitable implant from the variety of shapes and sizes at their disposal.

Simulation – Before Breast Augmentation Surgery

There is still no definite and effective way of simulating surgical results, or for correctly and accurately describing the postoperative size, shape and proportion of the breasts.
Digital simulations and placing substitute implants inside a special bra underneath clothing are deceitful in terms of final appearances and do not accurately showcase the breast’s final size, shape and proportions.
Over the years, the breast will undergo many changes during various stages, such as menstruation, pregnancy, lactation, weight loss / gain, etc., all of which affect a woman’s body and her breasts.
In fact, there is no substitute for a surgeon’s experience, skill and ability when it comes to choosing the right implant, and it is best to consult with the surgeon and take note of his advice.
From my experience, after having performed hundreds of breast augmentations, I can honestly say that most women come to see me because of my extensive experience in selecting the best implant size, type, shape and placement for each and every woman.
To conclude, I must add that this surgery gives the woman beautiful breasts, with impressive size and shape, that harmoniously match her body type, add grace and beauty that greatly contribute to positive body image, her confidence and make her feel joyous and more feminine.

Preoperative Tests

1. Before the surgery, please obtain all test results as requested by Dr. Govrin during your consultation.
2. It is recommended that every woman undergo an ultrasound of the breasts before the surgery, especially if you are over 30.
3. For women 35 and older, it is recommended that you also undergo a mammogram before the surgery, in addition to the ultrasound.

Anesthesia

Breast augmentation is performed under local or general anesthesia. It takes around 30 – 40 minutes to complete.
The woman may return home a few hours later.
This is a relatively simple procedure on the woman, and she may resume regular activity within a few days.

Breast Augmentation Surgery

In breast augmentation procedures, the surgeon inserts an implant into a pocket that he makes between the breast and the pectoralis major muscle (sub-glandular placement), or between the pectoralis major muscle and the ribs (sub-muscular placement).
There is also an option to combine both methods, with the implant partially above and partially below the muscle, which combines the benefits of both methods.

Sub-muscular placement is usually more painful and restricts daily activity for a longer period after surgery.
Dr. Govrin will choose the method based on the woman’s wants, the shape of her body and breasts and the advantages each method offers toward achieving the desired appearance.
The implanted is inserted through an incision made in the fold beneath the breast, around the areola or through the armpit.
Each of these incisions has its own advantages and disadvantages.
The best method for each woman will be chosen upon consulting with Dr. Govrin.

Results

Results are only visible a few weeks after the surgery. Please be patient throughout the recovery and healing period until final results are visible.
You will be asked to come in for a follow-up after the surgery.

Postoperative Instructions

You may feel nauseous or weak after the surgery, and this is because of the anesthetics administered during surgery.
These symptoms usually subside within two-three days after the surgery.
During the first few days after the surgery, your breasts will be stiff, and the skin will feel tight, accompanied by swollen breasts and a slight burning sensation around the areola.
You will experience some relief a few days later, though some sharp or stabbing pain in one or both breasts is normal.
These symptoms may appear once again before your next menstrual cycle.
This happens because of the implant stretching the tissue, but subsides shortly after.
You may use the analgesics prescribed by Dr. Govrin to alleviate any discomfort and pain.
You must take the oral antibiotics as prescribed by Dr. Govrin.
Shower every day, and even wash your breasts and the adhesive strips covering the stitches.
Three days after the surgery, remove the adhesive strips while showering.
You may go to the beach and tan (also using sunbeds) a month later.
After the surgery, it is recommended that you use Dr. Govrin’s Triple Therapy.
Dr. Govrin developed the Triple Therapy based on his extensive experience and it helps improve
skin texture, makes scars heal faster, and helps maintain surgical results for longer.

Support Bra

A supportive bra is crucial for women who have undergone breast augmentation.
Larger breasts weigh more. The breast’s weight strains the tissues and can lead to sagging and drooping breasts.
To maintain the shape and beauty of your breasts over time, it is important that you wear a support bra.
A support bra has tight, wide straps to help support the breast and maintain its shape and beauty for longer.
Dr. Govrin will recommend the right bra for your breasts.

You may resume physical activity within 3 – 4 weeks with the correct support bra!

Side Effects

When you choose skilled and experienced hands to perform your breast augmentation, side effects are usually rare, but include pain, bleeding, infection, breast or nipple numbness, asymmetry, and scar tissue.
You may also experience symptoms that have to do with the implant, such as folds, displacement, rejection, wear and tear and difficulty undergoing mammograms.
As mentioned, these side effects are rare after breast augmentations and can usually be avoided, especially when the surgery is performed by a knowledgeable and experienced surgeon.

When to Seek Urgent Medical Attention

1. Fever of 38 o C (100.4 o F).
   2. Severe redness on or around the breast.
   3. Sudden swelling of the breast.
   4. Fluid discharge from surgical wound.
   5. Breast suddenly hardens after softening.

Periodic Postoperative Tests

Every woman must get a routine breast exam performed by a breast surgeon, regardless of the surgery or its type.
